Assault with a treadly weapon
Cleaning and adjusting my mountain bike on the Aldi workstand, kneeling at the rear derailleur when the clamp post slid off and the bike fell on me - the saddle edge caught me right on the bonce! Stars were seen.

I think mostly user error - need to keep the stand on level ground in future...
